00 (based on experience) The Village at Victory Lakes, a part of Franciscan Ministries, is hiring a Social Services / Discharge Planner.
The primary purpose of this position is to assist in the management of the day-to-day operations of the Social Services department in accordance with current federal and state regulations, as well as campus policy.
Responsibility encompasses our care facility with skilled, ICF and specialty unit levels of care, as well as determination of appropriate resident service placement for new admissions and existing residents on campus.
This position requires a bachelors degree in Social Work or related field.

The Village at Victory Lakes, 1055 E Grand Ave, Lindenhurst, IL 60046 Duties and
Responsibilities:
Medicare, Medicaid, insurance and private pay discharge planning needs; working closely with all involved clinical disciplines, physicians, outpatient agencies, and families to ensure a safe discharge plan and smooth transition from our skilled healthcare facility.
Supports all level of care within the Victory Lakes campus and makes referrals as appropriate.
Maintain a high standard of resident care Attend a minimum of 12 annual hours of in-service education Promote and respect the dignity and well-being of each resident and their family Required skills and
Qualifications:
Two years of clinical experience in social services, and specialty skills specific to geriatric, but not required Must enjoy working with the senior population The Village at Victory Lakes is a vibrant senior living community in Lindenhurst, Illinois.
The Village at Victory Lakes is a continuing care retirement community (CCRC), offering a variety of senior lifestyle options including independent living, assisted living, memory support assisted living, and skilled nursing/long-term care.
